PUBLIC NOTICE OF MEDICAL RECORD DESTRUCTION

Northern Berkshire Pediatrics is hereby giving notice that our practice will be destroying paper medical records. The records scheduled for destruction will include all paper charts (records) from this practice created prior to the implementation of our Electronic Medical Record (EMR) System in August 2006. These charts include notes of visits, test results ordered by all current and past pediatricians of Northern Berkshire Pediatrics, and specialists that their patients were referred to.

All paper medical records have been scanned into our EMR System. If desired, the paper charts will be available for the patient to pick up prior to destruction.

If the child is a minor, the parent or guardian must sign a consent for release of information and present a photo ID to pick up the chart. Anyone 18 years of age and older must sign a consent for release of information and present a photo ID to pick up his or her own record.

 To request records, patients can call our office to request a release form or may download the form from our website.

 Patients may request records from June 1, 2019 until June 30, 2019.

 Records will be available for pick up approximately two weeks after request is received.

 Request made after the deadline of June 30, 2019 will not be processed.
